THE COMPARISION OF CLINICAL FEATURES OF ACUTE COVID-19 INFECTION AMONG OLDER AND YOUNG PATIENTS AT GERIATRICS AND PALLIATIVE CARE DEPARTMENT UNIVERSITY MEDICAL CENTER AT HO CHI MINH CITY

Abstract
Objectives: This study aimed to compare clinical characteristics of Covid-19 infection in older and young patients hospitalized at the Department of Geriatrics - Palliative Care, University of Medicine and Pharmacy Hospital, Ho Chi Minh City.
Methods: A prospective study was conducted in all patients with diagnosis of Covid-19, at Geriatrics and Palliative care department in UMC, from 07/2021 to 10/2021. Data on demographic, clinical, paraclinical aspects, treatments and complications were collected.
Results: The study conducted on 296 patients with diagnosis of Covid-19 infection. There were 47.2% (140) people under 60 years old and 52.7% (156) people aged 60 years and older. Compared with the group of young people, the older group had more statistically significant symptoms of sore throat (19.9%), fever (73.1%), delirium (8.3%), fatigue (75.6%), higher rates of comorbidities (60.9%) and more severe Covid-19 infection (50%). The group of older people with lesions on chest X-ray (80.1%), chest CT scan was higher. They also required higher oxygen support through canula (58.3%), mask (29.5%) than the group of young people with statistical significance. Older people required more specific treatment with Remdesivir (61.5%) and Baricitinib (10.9%). The complications such as ARDS (17.3%), bleeding (5.2%), and death in the older group accounted for 35 cases (22.4%), higher than in the young group (only 5 cases, accounting for 3.6%) with statistical significance.
Conclusions: Older people infected with Covid-19 have more severe clinical characteristics and higher risk of progression than younger people. A comprehensive geriatric assessment, early effective comprehensive treatment, and an appropriate prognosis are needed for the patient and the patient’s family.
